Does exercise reduce visceral (belly) fat?

Yes — moderate-to-high-intensity aerobic exercise, and time-efficient HIIT, reduced visceral belly fat in overweight adults, even without dieting and even when the scales barely moved. Two conditions: the intensity has to be real, and you cannot choose where the fat comes off.

Where this comes from

Why it works
Exercise raises the body's energy demand and prompts it to draw on fat stores. Visceral fat — the deep fat around the liver and gut — is metabolically active and turns over quickly, so it is often among the first stores to shrink. Because the effect runs through overall energy use, it cannot be aimed at one area, and it can happen even while the fat just under the skin, and the number on the scale, change little.
What the human evidence shows
The anchor is a confirmed meta-analysis of 15 exercise-only trials in 852 adults, with visceral fat measured directly on CT or MRI scans and no calorie restriction involved. Aerobic training significantly reduced visceral fat (effect size −0.55), and across the five controlled studies the pooled reduction was about 42 cm².1 Intensity mattered: only moderate and high intensity were significant, low intensity was not, and resistance training alone was only borderline. A later HIIT meta-analysis of 39 studies found the same for short, intense sessions, and newer network analyses confirm several exercise types can work.
Where the evidence stops
The trials are in overweight adults, not a menopause-specific group — the midlife shift towards central fat makes this relevant, but the studies were not run in perimenopausal cohorts, so read it as broadly applicable rather than tailored. There is no spot reduction, the effect needs genuine moderate-to-high intensity, and while pairing exercise with diet gives a larger change, the effect here stands on exercise alone.

A note on the scales: because visceral fat can fall while scale weight holds steady, the scale is a poor way to judge this.
